Free A1c Screening
It is recommended that A1c is tested every three months to make sure that your blood sugar is in line with your diabetes care plan.
To apply for our free A1c Screening program, please fill out the following form and a Diabetes Care Guide will be in touch with you shortly regarding enrollment. All applicants will be considered.
