Diabetes Education Resources
The DF’s accredited education offers tools for blood sugar management from healthy eating, monitoring, being active and coping. We offer one hour workshops, Diabetes Self-Management Education and Support (DSMES) and tip sheets that you can provide to your patients. Professional educators offer this service. We will come to your office and provide workshops as well! Available in English and Spanish.
Financial Assistance Applications
The Diabetes Foundation offers financial assistance for medication, supplies, AIC screening and transportation to and from doctor’s appointments.
