The simplest way to remember Diabetes Foundation in your estate plan is by leaving a gift in your will.
It can be as simple as including the following language in your will:

“I give ______% of my residuary estate to Diabetes Foundation, Inc., 45 Whitney Road, 2M, Mahwah, NJ 07430. Diabetes Foundation’s tax identification number is 22-3551926.”
or
“I give $______ of my residuary estate to Diabetes Foundation, Inc., 45 Whitney Road, 2M, Mahwah, NJ 07430. Diabetes Foundation’s tax identification number is 22-3551926.”

Charitable Gift Annuities
A charitable gift annuity allows you to receive fixed payments for the rest of your life while simultaneously making a gift to Diabetes Foundation.

Life Insurance
Name Diabetes Foundation as a beneficiary of a policy or transfer ownership of a whole life policy to Diabetes Foundation.

Retirement Assets
The easiest way to leave the balance of a retirement account to Diabetes Foundation after your lifetime is to list Diabetes Foundation on your beneficiary form, provided by your plan administrator.
