The Diabetes Foundation is here to support you and your patients who have prediabetes and diabetes. With everything to manage after a diagnosis (including your practice), you needn’t navigate the myriad services or manage patient concerns and questions alone.
When you partner with the Diabetes Foundation, we can create better outcomes for people living with a diagnosis of prediabetes or diabetes together, especially those individuals who would benefit from all the free and affordable resources we offer.

PREVENTION IS THE BEST MEDICINE
Have You Screened Them for Prediabetes?
Four out of 5 people in the U.S. who have prediabetes don’t even know it. Knowing the risk factors—and being screened for prediabetes—puts patients in control. We encourage you to speak to your patients about getting screened to see if they have prediabetes and are at risk for developing diabetes. The Diabetes Foundation makes it easy for the uninsured or underinsured to get tested at no charge.
Tools and Free Resources for You and Your Patients
The Diabetes Foundation’s healthcare provider toolkit is a comprehensive collection of valuable resources to share with patients who have been diagnosed with prediabetes or diabetes. Our support materials help you improve your patients’ overall well-being by providing them with a pathway toward better health and diabetes self-management.
